This position is for a Registered Nurse providing home health care services primarily in Milwaukie, Oregon, delivering patient-focused care under physician orders. The RN coordinates care according to individualized patient needs and complies with nursing standards and regulations. The role requires relevant nursing licensure, BLS certification, and experience in acute care and preferably home health settings.

Schedule : M-F, Day shift, with rotating weekend coverage

Location : primarily Milwaukie and surrounding areas

Job Description:

The Registered Nurse Home Health, under the direction of the Home Health Clinical Supervision and on order of the physician, is accountable for direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Professional development is an integral part of this nursing practice. Position coordinates patient care activities with other members of the health care team in a collegial relationship.

Required Qualifications:

• Education to meet certification, license or registration requirement.
• National Provider BLS - American Heart Association within 30 days of hire.
• Oregon Registered Nurse License upon hire.
• 1 year - Medical/surgical experience in an acute care setting.

Preferred Qualifications:

• Recent Home Health experience.
• Community mental health experience, geriatric experience, and ANCC Psychiatric for Mental Health RN work assignment.
• Hospice or Palliative Care experience for Palliative Care RN work assignment.
• Point of Care experience.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
