Provides leadership and support to address individual patient needs, utilizing community and healthcare resources to ensure continuity of care post-discharge. Ensures timely and appropriate interventions to optimize resource utilization and patient outcomes.
Summary
JOB SUMMARY: Acts a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, re-admission to post-discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization i.e. length of stay and level of care. REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Graduate of a Master's degree program in social work, psychology, or other appropriate degree. Less than one year of experience. Maryland Social Work licensure LMSW or LGPC.Additional Information
