Job Title: Pediatric Diabetes Care & Education Specialist
Department: Pediatric Endocrinology

• Type: Full-Time (REMOTE/HYBRID Option Available)
• Shift: 5x8s – Days (8:00 AM – 5:00 PM)
• Daily Caseload: Minimum of 5 patient visits per day

The Pediatric Diabetes Educator plays a vital role in managing and educating pediatric patients with diabetes and their families. Working alongside a Pediatric Endocrinologist , this role focuses on patient-centered care, education, and empowerment to promote safe and effective diabetes management both in the clinic and at home.

The educator independently maintains a daily schedule, collaborating with endocrinology providers, nurses, and other care team members to ensure comprehensive and coordinated patient care.

• Maintain an independent daily schedule for diabetes management and education visits.
• Provide comprehensive education to pediatric patients and families on:
  • Disease understanding, insulin management, glucose monitoring, nutrition, physical activity
  • Use of diabetes technologies (e.g., insulin pumps, continuous glucose monitors )
• Conduct follow-up visits to assess skills, address barriers, and reinforce care plans.
• Collaborate with multidisciplinary teams (endocrinologists, dietitians, nurses, etc.) to ensure care continuity.
• Assist families in developing individualized diabetes care strategies for home, school, and extracurricular settings.
• Support families with insurance navigation, durable medical equipment, and prescription needs.
• Maintain accurate documentation in EMR in accordance with ADA, CMS, and institutional guidelines.
• Participate in quality improvement initiatives, program development, and ongoing education.
• Serve as a resource for staff education and best practices in pediatric diabetes management.

• Experience: Minimum 5 years of experience in diabetes care or pediatric endocrinology
• Education:
  • BSN or Registered Dietitian (RD) with a Master’s degree
• Certifications:
  • Certified Diabetes Care and Education Specialist (CDCES) – Required
  • Pump Training Certification (Omnipod and/or Tandem) – Preferred
• First-Time Travelers: Accepted ✅
